Good Faith Estimate (No Surprises Act)
Effective Date: November 12, 2025
At The Tempered Way Therapy LLC, we believe in transparency and fairness — no surprise bills, no hidden costs. Under federal law (the No Surprises Act, effective January 1, 2022), all health care providers are required to give clients who are uninsured or who choose not to use insurance a written Good Faith Estimate (GFE) of expected charges.
This helps you understand and plan for the cost of your care before services begin.
1. Your Rights Under the Law
You have the right to:
Receive a Good Faith Estimate explaining the expected total cost of your mental health services.
Request a GFE before scheduling, when inquiring about services, or any time during treatment.
Receive the estimate in writing, typically within 1–3 business days of your request or initial scheduling.
Dispute a bill if it is more above the amount listed on your Good Faith Estimate.
For more information about your rights, visit:
www.cms.gov/nosurprises
2. What’s Included in the Estimate
Your Good Faith Estimate will outline the anticipated total cost of services agreed upon between you and your therapist.
This may include:
Individual therapy sessions
Family or couples therapy sessions (if applicable)
Late cancellation or no-show fees
Any additional services (for example, letter-writing, clinical summaries, or consultations)
Your estimate is based on the information available at the time and is subject to change if your treatment plan, frequency, or needs evolve. Any updates will be communicated to you promptly.
3. Disputing a Bill
If you receive a bill that is more than your Good Faith Estimate, you have the right to dispute it.
You can start the dispute process by submitting a request to the U.S. Department of Health and Human Services (HHS) within 120 days of receiving the bill.
To learn more or file a dispute, visit www.cms.gov/nosurprises or call 1-800-985-3059.
Keep a copy of your Good Faith Estimate in your records. You may need it if you choose to file a dispute.
4. How to Request a Good Faith Estimate
You can request a Good Faith Estimate from us at any time—before, during, or after scheduling your first appointment.
We’ll provide it in writing (by secure email or client portal) within the legally required timeframe.
To request your estimate, contact:
The Tempered Way LLC

5. Important Notes
The Good Faith Estimate shows the expected charges for the services you plan to receive but is not a binding contract.
Actual costs may differ if your treatment plan changes or if additional services are requested.
You are encouraged to discuss any financial concerns openly — we believe therapy should be collaborative, transparent, and free from financial surprises.
6. Legal Reference
This notice is provided in accordance with Section 2799B-6 of the Public Health Service Act, 45 C.F.R. § 149.610, and Colorado Revised Statutes § 10-16-705, which require transparency for self-pay and uninsured clients.
Under federal law, you have the right to a Good Faith Estimate of expected charges. For details, visit www.cms.gov/nosurprises.
